She had recently been discharged from hospital and wished to spend her last days in the comfort of her own home. Following her discharge from the hospital, her health and symptoms, unfortunately, got worse. Due to her health deteriorating Kathy contacted the existing community services within her locality, but they were unable to assist her so she dialled 999 and was taken to her local emergency department.
HomeLink Healthcare’s at home services team were co-located within this acute Trusts’ emergency department. Our trained and knowledgeable staff were on hand, working collaboratively with the Hospital team, to help identify Kathy’s ongoing care needs and to provide the level of at home care she needed to return to her home safely. Once she had been assessed fully in the emergency department we were able to determine the level of care she would require from our nursing and physiotherapy team once she returned home. Kathy was discharged from hospital and sadly passed away 48 hours later in the familiar surroundings of her own home just as she had wanted.
