We deliver cost-efficient community-based models of care as an alternative to hospital inpatient care

Early Supported Discharge services and Virtual Wards offer patients the care they need at home, instead of in hospital. These services fill in the gap until established community services are available or deliver types of home-based care not provided locally. Utilising this approach helps reduce Delayed Transfers of Care (DTOC), freeing up inpatient capacity which can ease pressure on elective procedures and admissions from A&E. Our Admission Avoidance services provide clinical care at home as an alternative to hospital referral from primary care or admission through A&E.

HomeLink Healthcare works with NHS Trusts and their system partners to offer these pathways as a safe and high quality option for patients who welcome the opportunity of an early return to the familiar surroundings of home.

Clinical home care is provided by our highly skilled Multidisciplinary teams, supported by technology, delivering care at the right time in the right place. Our ‘in-reach team, based at the hospital, ensures tight integration with existing processes, identifies and assesses patients and manages the transition from hospital to home.

A wide range of nursing and therapy led services are available including:

  • Intravenous therapies including antibiotics
  • Medicine administration and management
  • Wound care
  • Post surgical rehabilitation and recovery
  • Physiotherapy
  • Occupational Therapy
  • Care monitoring
  • Bridging packages of care

Each service is bespoke to reflect local needs

We have been working with Norfolk and Norwich University Hospitals to release in-patient bed capacity and improve patient choice.

“HomeLink Healthcare believes that everyone should have the choice to be treated at home instead of in hospital.”
