Is a privately owned limited company established by senior healthcare professionals and experienced healthcare managers with the primary aim of delivering safe, effective, responsive, caring , well led and cost efficient models of care at home as an alternative to hospital-based inpatient care.
Early supported discharge
Under our Early Supported Discharge model and after a full assessment, patients are given the opportunity to continue treatment at home rather than in hospital. This helps the NHS Trust to free up beds as well as providing the patient with a more convenient location to receive their treatment.
HomeLink Healthcare can create a virtual ward with the potential of creating a 10, 20, 30 bed virtual ward to meet the demands of the NHS Trust.
During treatment, the patient remains under the care and accountability of the hospital consultants.
Under our Admission Avoidance model and after a full assessment, patients who attend Accident and Emergency may be suitable for treatment at home therefore preventing admission to a bed on a ward.
Our teams can also work closely with GP’s to provide treatment in the home, preventing the need for admission to hospital.
Patients can remain under the care of the hospital consultant or their own GP for the duration of treatment and receive high-quality patient-focused care at home.
